The head of a review into maternity care in the UK has warned the situation is "much worse" than she expected.
Initial findings published by Baroness Amos include concerns about "unacceptable" care and poor cleanliness on wards of National Health Service hospitals.
The full findings from the investigation are due to be published next year.
