A senior general in the US has said it will be difficult to maintain what he called 'extraordinarily high standards' to avoid civilian casualties in the Iraqi city of Mosul, which American-backed forces are fighting to retake from the so-called Islamic State group.
General Joseph Votel was speaking as the military begins a formal investigation into an explosion there that's believed to have killed scores of civilians.
