Chapecoense crash in Colombia was human error

Colombian aviation authorities have concluded that the plane crash that killed 71 passengers, including most of the Chapecoense Brazilian football team was caused by a series of human errors.

Investigators did not find any technical failures, but rather a series of missteps by the pilot, as well as Bolivian regulators which resulted in the cash.
