Problems with IV Line Affected Oklahoma Execution

A report released by the Department of Public Safety for the US State of Oklahoma has found that a botched execution there back in April was down to an improperly monitored intravenous line.

The review says the failure to monitor the line correctly probably caused problems in administering the three lethal injection drugs.
